NuLook Home Care Agency is seeking a compassionate, organized, and experienced CLS Case Manager to oversee individuals receiving Community Living & Supports (CLS) services.
This position is ideal for someone with a background in behavioral health, social work, human services, psychology, case management, intellectual/developmental disabilities (I/DD), or a related field who understands person-centered care and enjoys helping individuals become more independent and engaged in their communities.
Key Responsibilities
The CLS Case Manager will:
- Manage an assigned caseload of individuals receiving CLS services.
- Review each client's ISP/person-centered goals, authorization, and service needs and ensure services remain aligned with those goals.
- Develop and maintain aide care plans/support instructions based on the client's ISP and authorized goals.
- Conduct required client follow-ups, including 90-day reviews and other scheduled monitoring.
- Monitor client progress toward independence, community integration, skill development, and other established goals.
- Communicate regularly with clients, guardians/families, caregivers, care managers, and other members of the individual's support team.
- Monitor CLS staff performance and ensure caregivers understand the client's goals, behavioral needs, safety concerns, and support strategies.
- Identify changes in client needs, behavioral concerns, service issues, hospitalization, incidents, or other matters requiring escalation.
- Ensure service documentation is complete, timely, accurate, and consistent with the client's authorized services and goals.
- Participate in ISP/team meetings and coordinate follow-up actions when needed.
- Assist with resolving service concerns, caregiver issues, scheduling needs, and gaps in care.
- Work closely with NuLook's clinical, administrative, compliance, and staffing teams to maintain quality and continuity of services.
- Maintain organized, audit-ready client records and follow applicable Medicaid, Alliance Health, and agency requirements.
Qualifications
We're looking for someone with education, training, or professional experience in behavioral health or human services. Strong candidates may have experience or education in:
Social Work • Psychology • Behavioral Health • Human Services • Case Management • Counseling • I/DD Services • Mental Health • Special Education • Community-Based Services
A bachelor's degree in a human-services-related field is preferred, but we can consider candidates with a strong combination of relevant education, training, and direct experience.
Candidates should also have strong documentation and communication skills, be comfortable working with individuals with behavioral or developmental support needs, understand confidentiality and professional boundaries, and be able to manage multiple clients and deadlines independently.

The right Case Manager will understand what each person is working toward, recognize when services aren't matching the ISP, communicate with families and staff before small issues become larger ones, and help ensure CLS is producing meaningful progress—not simply filling authorized hours.
